By signing below, I (the Customer) hereby authorize JDS Appliance Repair Service ("JDS") and its assigned technician(s) to enter my property at the address listed above on the scheduled service date, without my physical presence, to perform the requested appliance repair service.
I understand and agree to the following:
- I am voluntarily granting permission for unattended entry into my home.
- JDS technicians will only access the areas of the home necessary to perform the repair.
- JDS Appliance Repair Service shall not be held liable for any pre-existing property damage, loss of personal property, or any incidents that may occur as a result of this authorized unattended entry.
- JDS carries general liability insurance; however, this authorization waives claims arising solely from the unattended nature of the visit.
- I have provided accurate access instructions and take responsibility for ensuring the technician can safely enter and exit the property.
- This authorization is valid for the scheduled service date only and does not constitute a standing permission for future entries.
